We aim to contact people within five working days. However, if the request needs to be dealt with in the next 24 hours tick the box below.

Medical conditions

Include all relevant conditions and date / year of diagnosis; alternatively upload a recent medical summary.

Attachments

Only the following file types can be uploaded: Word/JPG/GIF/PNG/PDF. There is a 2Mb limit per attachment.

Files must be less than 50 MB.
Allowed file types: gif jpg jpeg png txt rtf html pdf doc docx.
Files must be less than 4 MB.
Allowed file types: gif jpg png txt rtf html pdf doc docx.
