Children's social care request for service form

This form, known as the MASH enquiry, is for professionals to refer to Children’s Social Care.

If an immediate response from Children’s Social Care is necessary, call 0345 050 7666. If you have an urgent concern for a child call 999 or 101.

This page is for professionals only to make children's social care referrals. If you are a member of the public with a concern for a child call 0345 050 7666 or use this form.

No immediate risk or harm

If your referral does not indicate immediate risk of, or actual, significant harm, the completion of a strengths and needs form is requested to further explore concerns/support needed as an alternative to a safeguarding referral.

Level of need

If the case is a level 1 or 2 you need to contact the Locality and Community Support Service (LCSS) for advice.

Cases at level 1 or 2 will not be accepted into the MASH.

There are multiple teams to cover all areas of Oxfordshire, you will find link worker names and contact numbers on the Oxfordshire Safeguarding Children's Board website
