Urgent response

Countywide service providing urgent response to social and healthcare crises in the community.

Aim of the service

The urgent response service will provide emergency support to service users and carers in a social care crisis which would otherwise result in a stay in hospital or a care home, or otherwise pose a serious threat to the person’s safety, health or wellbeing.

Crisis situations will primarily be those relating to an unexpected change in someone’s personal circumstances.  For example, an older person having a fall, or a carer being unexpectedly admitted to hospital, leaving the person they care for without necessary support. 

Following acceptance of a referral the service will comprise:

  1. A first visit of up to four hours to include an assessment of ongoing need.
  2. Further home support of up to five days

Following this the person will either be ready for discharge or move to the reablement service.

NB. there is no contractual arrangement in this service for a person to stay longer than five days.
