You can refer a patient for reablement services via the Home First Referral form.

What is reablement?

Reablement is short-term support provided by a group of specialists to help a patient at home regain independence.

It is centred around helping with practical daily tasks and activities with an assessment based on what you can do now, what you aim to achieve and how we can help get you there.

Who is eligible?

You must either pay your council tax to Oxfordshire County Council or be registered at a GP in the county.

You also need to meet the following criteria:

  • Be medically stable.
  • Have the potential to gain independence if supported.

How long does the service last?

This depends on the assessment given by the team of professionals and the goals set to help you regain your agreed level of independence.

Once completed, if you can manage on your own, you can find a range of information and support on Live Well Oxfordshire.

If you cannot, we will work with a team of specialists to build a plan for your longer-term care or support.

Who delivers the service?

The service is delivered by a combination of

  • our adult health and social care team
  • the Home First specialists
  • a network of providers.

How much does it cost?

This service is free. After it has been completed, we'll complete an assessment of your ongoing care needs if you have them. A financial assessment will establish if you need to contribute towards the cost of your care. If you are expected to contribute, it will start from the date we identify you need long term care and support.
