Nomination (or Expression of Wish) Form

 Being a member of the Local Government Pension Scheme (LGPS) can, in certain circumstances, provide a lump sum payment in the event of your death. The exact amount of the death grant will depend on your personal circumstances, including whether you have other LGPS records.

Use this form to nominate who you would like to receive a lump sum in the event of your death. 

It is much quicker and easier to complete the form online; head to Oxfordshire Pension Fund to register for or log in to our secure online service.

The Oxfordshire Pension Fund (the Fund), has absolute discretion in paying the death grant. This means the Fund gets the final say in who receives the money, although they will be guided by your wishes.  You can cancel or change your expression any time in writing or online and the Fund will respect your wishes as far as possible, but the final decision about payment of the Death Grant will rest with the Fund.

Option to elect to the 50/50 section of the scheme

Use 50/50 election form to elect to halve your contribution. Maintain scheme continuity and reduce your costs, and while paying less build up your pension at half rate. A temporary arrangement by election to your employer.

Rejoin the LGPS main section

If you choose to return to the main scheme use Re-join the LGPS main section form to change the election.

Opt out of the scheme

If you decide to opt out of the LGPS, you will need to complete the Opt Out form (pdf format, 490Kb) and send to your employer’s payroll department. We recommend you read the entire form and get independent financial advice before completing the form.
